Purpose of the Scholarship: Creative Bioscience, LLC, its officers, directors, employees, agents, and affiliated companies ("Creative Bioscience") notifies all interested college students of its Annual Fitness Scholarship. The Fitness Scholarship is designed to teach, encourage and promote a healthier lifestyle and healthier eating habits at an early age. The amount of the scholarship is $1,000.00 and it is awarded once per year on January 15th. As the industry leader for diet supplements and diet support service, our business success depends on maintaining our reputation as the most reputable provider of natural, clinically supported, and safe diet supplements.


Agreement to Application Requirements: By applying for this scholarship, you agree to the application requirements set forth herein.


Eligibility: The scholarship is open to legal residents of the United States of America, who are 18 years of age or older as of November 1, 2012, and who are not employees, family members, or relatives of Creative Bioscience.


Application Deadline: You must complete your application by November 1 of the preceding year for the January award.


Application Process:


a. Registration: You must register online at www.creativebioscience.com, and submit your name, address, telephone number, email address, proof of legal residence, and proof of full time student status and active registration with your school.


b. Essay: You must submit an original 300 to 500 word essay. Write an essay about any subject that interests you in the health industry, focusing on why it interests you and how you have learned from it, or could apply it into your life. Reference the content on the website that your are writing your essay about. Essays must be original and written by the applicant. We check essays for plagarism, re-writes, and duplication.


c. Submission: You must submit your essay with your registration information at www.creativebioscience.com.


d. Review: Between November 1st and January 15th, we review the essays for content, organization, grammar, spelling, and creativity.


Selection: after you complete the 3 month trial, you summarize your experience with a final post.


Notification: On or about January 15th, Creative Bioscience® notifies the recipient of the scholarship with the contact information provided. If, within 14 days, Creative Bioscience® does not receive a response from the recipient, then, at its sole discretion. Creative Bioscience® reserves the right to award the scholarship to the applicant who is the author of the next best essay.


Claiming the Scholarship: To claim the scholarship, the recipent must provide proof that they are legal residents of the United States of America, proof that they are 18 years of age or older as of November 1, 2012, proof of full time student status with active enrollment documentation, and an address at which they may receive ceritified, insured, and return receipt restricted U.S. mail. If, within 14 days, the recipient fails to claim the scholarship, then, at the sole discretion, Creative Bioscience® reserves the right to award the scholarship to the applicant who is the author of the next best essay.


Affidavits: As its sole discretion, Creative Bioscience® may require the scholarship recipient to complete, execute and return attidavits of eligibility, liability releases, and publicity releases bore taking possession of their application prize. If the recipient fails to sign these documents, if a recipient cannot be notified, if a recipient is found to be ineligible, and/or if a recipient is found not to be in compliance with this fitness scholarship application, then, at its sole discretion, Creative Bioscience® reserves the right to disqualify the recipient and award the scholarship to the applicant who is the author of the best essay.


Promotional Rights: In a writing, the scholarship recipient must grant Creative Bioscience® the right to use and publish their name, biographical information, statements made regarding Creative Bioscience, or the promotion, portrait, picture, voice and likeness for trade, advertising and promotional purposes, without additional consideration, notification, review, and/or approval, worldwide, and/or on the internet.


Disqualification: At its sole discretion, Creative Bioscience® reserves the right to disqualify applicants who Creative Bioscience® believes or has reason to believe is violating the terms and conditions of this scholarship application.


Taxes: Any and all applicants are solely responsiblefor any and all applicable state and/or federal taxes, and/or any and all other fees associated with this scholarship.


As is: Everything in this scholarship application, is provided as is, without warranty of any kind, express or implied, and/or including, but not limited to merchantability, fitness or a particular purpose, and/or non infringement.


Cancellation: At its sole discretion, Creative Bioscience® reserves the right to cancel this scholarship at any time without penalty.


Release, Indemnification, and Hold Harmless: By applying for this scholarship, applicants agree to release, indemnify, and hold harmless Creative Bioscience® from any and all liability for any and all claims that may arise from applying for this scholarship.


Jury Trial Waiver: Applicants waive any aand all rights they may have to any and all trials by jury.


Law, Jurisdiction, and Venue: The laws of the State of Utah govern the interpretation of this scholarship application without giving effect to the choice of law provisions thereof. The Courts of the State of Utah possess exclusive jurisdiction over any all disputes which may arise from this scholarship application. The Third Judicial District Court, State of Utah, Salt Lake County, Salt Lake City Department is the exclusive venue for any all legal, judicial, administrative, equitable, and/or any other claims which may arise from this scholarship application.


Integration Clause: This scholarship application constitutes the complete and final agreement, supersedes all applications, is the full and exclusive statement of the official scholarship application, and may only be modified in a writing by Creative Bioscience.
